The Transporter named above is authorized to load, haul, and unload the horse for this Authorization. No other person may transport the horse under this Authorization without the Owner's further written consent.

4. Scope — Per-Trip or Blanket Authorization

The Owner elects (check one): ☐ PER-TRIP — this Authorization covers only the single trip described in Section 2 and expires when that trip is complete; or ☐ BLANKET — this Authorization covers all transport of the horse by the Transporter from [START DATE] through [END DATE], and the Transporter will record the origin, destination, date, and purpose of each trip. A blanket authorization may be revoked by the Owner in writing at any time; revocation does not affect a trip already under way.

5. Assumption of Risk and Owner Acknowledgment

The Owner understands that trailering and transporting a horse are inherently dangerous and that a horse may be injured or killed despite careful handling. A horse may panic, scramble, kick, rear, fall, or injure itself while loading, hauling, or unloading; may be hurt by sudden stops, traffic accidents, road or weather conditions, or equipment or tire failure; and may colic, tie up, overheat, or suffer other stress-related illness in transit. The Owner knowingly and voluntarily assumes these inherent risks of transport.
